In this article, the use of ultrasound-assisted spinal anesthesia in patients with interspinous spacers is explored. Interspinous spacers are implants used in the treatment of lumbar spinal stenosis, but their presence can obstruct the path of the spinal needle during anesthesia administration. The case presented in the article demonstrates how lumbar neuraxial ultrasonography can aid in the safe and successful administration of spinal anesthesia in patients with challenging anatomy, such as those with interspinous spacers. This innovative approach offers a solution to a potential complication in anesthesia administration for patients with lumbar interspinous spacers
