Spontaneous pneumomediastinum is an unusual medical situation in pediatric subject. We report two pediatric circumstances with first time wheezing episode difficult by pneumomediastinum. Investigations didn’t reveal any underlying trigger for secondary pneumomediastinum. Pneumomediastinum mostly happens in asthmatic youngsters. It may be defined by elevated stress gradient between the intraalveolar and interstitial areas. We conclude that prime prevalence of respiratory infections in youngsters predisposes for spontaneous pneumomediastinum as a consequence of elevated stress inside obstructed airways, or by tissue necrosis from parenchymal an infection.
