Congenital anomaly of the posterior arch of the atlas: a uncommon threat issue for posterior circulation stroke.

Summary
We report a particularly uncommon case involving a posterior arch anomaly of the atlas, inflicting a vertebral artery dissection (VAD) induced posterior circulation stroke. A 16-year-old woman was admitted to hospital due to new onset dizziness. VAD associated a number of infarction within the posterior circulation was revealed. The congenital posterior arch anomaly of the atlas, together with instability of the atlantoaxial joint, had been found by chance throughout follow-up. That is the primary case of ischemic stroke associated to atlantal posterior arch aplasia and atlantoaxial instability. Though uncommon, it reminds us that investigation of the craniovertebral junction must be thought-about when stroke happens in younger sufferers.
